Medicare Chronic Condition Management Plan (CCMP) – FREQUENTLY ASKED QUESTIONS
If your GP has referred you to GutCode under a Chronic Condition Management Plan (CCMP), you may be eligible for a Medicare rebate for your appointment.
Please read the information below before your appointment and before making payment.
What is a CCMP and how many appointments can I claim?
A CCMP allows eligible patients to access Medicare rebates for up to 5 allied health services per calendar year (1 January–31 December).
These 5 services are shared across all eligible allied health practitioners – they are not 5 services per practitioner. For example, if you have used 2 services with a physiotherapist, you may have 3 remaining for other allied health services.
It is your responsibility to keep track of how many CCMP services you have used. GutCode cannot access your Medicare claiming history or confirm how many services you have remaining. If you are unsure, please check directly with Medicare.
How much is the Medicare rebate?
The Medicare item number for an eligible dietitian consultation is 10954. The standard Medicare rebate is currently $63.40 per eligible consultation and may be higher if you have reached the applicable Medicare Safety Net threshold.
How do I claim my rebate?
GutCode cannot process Medicare claims directly because our practice software does not integrate with Medicare. You will pay for your consultation as usual and then submit the claim yourself.
Claims through Medicare Online generally take less than 3 minutes:
- Find your GutCode receipt. It will be emailed to you on the date of payment and is also available under Billing/Payments in your client portal.
- Log in to Medicare through myGov or the Express Plus Medicare app.
- Select Make a Medicare claim and enter the requested details.
- Upload a clear photo or screenshot of your GutCode receipt and submit the claim.
Medicare will deposit the rebate into your nominated bank account.
Your Medicare receipt will contain the information required for your claim, including item number 10954, provider details, referring doctor’s details and referral date.
If you cannot claim online, we will also provide a copy of the Medicare Claim form (MS014) for manual claiming.
Medicare or private health – please decide before payment
You cannot claim the same consultation through both Medicare and private health insurance.
You must decide before payment whether you are claiming through Medicare using your CCMP or through private health insurance.
The receipt, item number and provider information required for Medicare are different from those required for private health insurance. We therefore need to know which option you are using before your payment is processed and your receipt is generated.
Once the receipt has been generated, we cannot change it from a Medicare receipt to a private health receipt, or vice versa.
Claiming through Australian Unity?
Please let us know before payment if you are claiming through Australian Unity, as they require different provider/business details from our standard private health receipt.
Before your appointment
Please ensure GutCode has received your current GP referral, decide whether you are claiming through Medicare or private health insurance, and let us know your choice before payment.
If using Medicare, it is your responsibility to ensure you have an eligible CCMP service remaining for the current calendar year.
Medicare rebates and eligibility requirements may change. GutCode provides the appropriate consultation receipt but cannot guarantee Medicare eligibility or payment of a rebate.

GutCode Artificial Intelligence (AI) Use Policy
Purpose
GutCode uses selected artificial intelligence (AI) technologies to support clinical documentation, communication, administrative processes and the efficient operation of the practice.
This policy explains how AI is used at GutCode, how client information is managed when AI tools are used, and the responsibilities of GutCode and its practitioners.
AI is used as a supporting tool only. It does not replace clinical judgement, professional responsibility, clinical decision-making or appropriate human oversight.
Use of AI During Consultations
GutCode currently uses Heidi, an AI-assisted clinical documentation tool, during consultations. Heidi may be used to assist with:
- capturing relevant information discussed during a consultation;
- supporting clinical note-taking;
- preparing draft consultation notes;
- preparing client summaries and plans; and
- assisting with correspondence to GPs, specialists and other healthcare professionals.
The use of Heidi is in addition to the practitioner’s own clinical note-taking and documentation processes. It does not replace them.
Any AI-generated notes, summaries or correspondence are reviewed and, where necessary, edited by the treating practitioner before being incorporated into the clinical record, provided to the client or sent to another healthcare professional.
The treating practitioner remains responsible for the accuracy, relevance and appropriateness of the final documentation and correspondence.
Client Information and Data Minimisation
GutCode takes a data-minimisation approach when using AI technologies and limits the identifying information provided to these systems.
A client’s full name is never entered into or intentionally provided to AI tools used by GutCode.
A client’s first name may be used where required to support natural and accurate documentation.
Unnecessary personal identifying information is not intentionally provided to AI tools.
Where AI is used to assist with correspondence to another healthcare professional, the name of the relevant GP, specialist or healthcare professional may be included where necessary.
Although direct identifiers such as a client’s full name are not provided, information discussed during a healthcare consultation may contain clinical or contextual information that could potentially identify an individual. GutCode therefore treats information processed by AI-assisted clinical tools as confidential health information and applies appropriate privacy safeguards.
Information Provided Before Your Initial Consultation
New clients are provided with information about GutCode’s use of AI before their initial consultation, including access to this policy.
The New Client Form also provides clients with an opportunity to indicate that they do not want AI assisted documentation used during their consultations.
The New Client Form is not the only way a client can decline AI use. Clients may also advise GutCode directly before an appointment or decline when asked by the practitioner.
Not all clients complete the New Client Form, and completion of the form is not relied upon as the sole method of determining AI preference.
Clients do not need to consent to AI use through the New Client Form.
Leaving the AI opt-out box unticked does not constitute consent.
Where a client has not opted out, the treating practitioner will obtain consent before commencing AI assisted documentation during the consultation.
Consent to AI-Assisted Documentation
Where a client does not have an existing AI opt-out recorded, the treating practitioner will explain the proposed use of Heidi and obtain consent before commencing it.
Consent is obtained for each consultation in which Heidi is proposed for use.
Receiving this policy, leaving the New Client Form opt-out box unticked, or attending an appointment does not automatically constitute consent to AI use.
Clients are under no obligation to consent.
Declining AI use will not affect access to GutCode’s services, the consultation provided or the standard of care received.
If You Prefer Not to Use AI
Clients may advise GutCode that they do not want AI-assisted documentation used.
This can be done:
- through the AI opt-out option on the New Client Form;
- by advising GutCode before an appointment; or
- when asked by the practitioner before AI is commenced.
Where a client declines, GutCode records this preference in the client’s clinical notes so that it is clearly visible to the practitioner.
The preference is treated as a standing opt-out.
AI-assisted documentation will not be used while this preference remains in place, and the client will not be repeatedly asked to reconsider their decision at future consultations.
Clients do not need to provide a reason for declining.
If a client subsequently wishes to change their preference, they may advise GutCode at any time.
Withdrawing Consent
Clients may ask for the AI-assisted documentation tool to be stopped at any point during a consultation. If this occurs, the consultation will continue using the practitioner’s usual clinical note-taking processes.
Clients can also advise GutCode whether they would like their decision to apply as an ongoing opt-out for future appointments.
Recording Consent and AI Preferences
The treating practitioner documents consent when AI-assisted documentation is used during a consultation.
Where a client has declined AI use, this preference is recorded in the client’s clinical notes so that it is visible before future consultations.
An absence of a recorded AI opt-out does not mean that consent has been provided. It means only that GutCode has no existing record that the client has declined AI use.
Where there is no existing opt-out, the treating practitioner obtains consent before commencing AI assisted documentation.
Audio, Dictation and Temporary Files
When Heidi is used during a consultation, audio or dictated information may be temporarily processed for the purpose of assisting with clinical documentation.
These files are used to assist with the preparation of relevant notes, summaries or correspondence.
Temporary audio or dictated files under GutCode’s direct control are deleted once they are no longer required for their intended purpose.
Final practitioner-reviewed clinical notes and relevant correspondence are retained as part of the client’s clinical record in accordance with GutCode’s usual record-keeping and privacy practices.
Clinical Responsibility and Human Oversight
AI does not independently make clinical decisions at GutCode.
AI does not replace the practitioner’s:
- clinical assessment;
- professional judgement;
- interpretation of clinical information;
- individualised nutrition recommendations;
- treatment planning;
- communication with the client; or
- responsibility for client care.
Information generated by AI is considered supporting or draft material until it has been appropriately reviewed.
The treating practitioner remains responsible for all clinical decisions and recommendations provided to the client.
Review of AI-Generated Information
AI systems can occasionally produce incomplete, inaccurate or inappropriate information. For this reason, GutCode does not assume that AI-generated information is accurate.
The treating practitioner reviews AI-assisted clinical documentation, client summaries, recommendations and professional correspondence and corrects material inaccuracies or omissions before the information is relied upon as final clinical documentation.
Other Uses of AI Within GutCode
GutCode may also use AI and automation technologies outside client consultations to improve business efficiency, administrative processes and the quality of its services.
AI may assist with:
- administrative processes and internal workflows;
- drafting and editing written material;
- development of templates, policies and procedures;
- organisation and summarisation of information;
- business planning and analysis;
- development of educational materials and resources; and
- other appropriate operational activities.
The same principles of privacy, data minimisation and appropriate human oversight apply to these uses.
Client full names are not entered into AI tools used for these purposes.
GutCode seeks to avoid providing unnecessary identifying client information to AI platforms.
Administrative and Practitioner Responsibilities
GutCode’s administrative team is responsible for ensuring new clients are provided with access to information about the practice’s use of AI.
The New Client Form provides clients with an opportunity to opt out in advance. If a client communicates an opt-out through the form or directly to GutCode, this preference is recorded in the client’s notes for the treating practitioner.
Administrative staff are not responsible for obtaining consent to the use of AI during the booking process and do not need to routinely ask clients for their AI preference.
Where there is no existing opt-out, the treating practitioner is responsible for obtaining consent before commencing AI-assisted documentation.
The treating practitioner is also responsible for appropriately documenting consent, respecting a client’s decision to decline or withdraw consent, reviewing AI-generated information and retaining responsibility for all clinical decisions and care.
Privacy and Confidentiality
GutCode recognises that health information is sensitive and takes reasonable steps to protect client privacy and confidentiality when selecting and using AI technologies.
The use of AI does not reduce GutCode’s existing responsibilities regarding privacy, confidentiality, clinical record keeping and information security.
AI technologies are intended to complement, rather than replace, GutCode’s existing clinical governance and privacy processes.
This policy should be read alongside GutCode’s broader Privacy Policy.
Questions or Concerns
Clients are encouraged to ask questions about the use of AI if they would like further information before providing consent.
Questions or concerns about AI, privacy or the handling of personal information can be raised with GutCode at any time.
Choosing not to use AI-assisted documentation will not affect the care provided.
Policy Review
AI technology and the professional and privacy requirements surrounding its use continue to evolve.
GutCode will periodically review this policy, the AI technologies used within the practice and its associated procedures to ensure they remain appropriate and consistent with relevant privacy, professional and clinical obligations.
The policy may also be reviewed whenever there is a material change to how AI is used within GutCode.
